The Empowerment Passport™ is a digital platform that helps individuals with long-term health conditions or disabilities clearly communicate their personalised adjustment needs across various life stages—education, employment, and other transitions.

Why an Empowerment Passport?

    The Empowerment Passport simplifies this process by giving individuals a structured, respectful way to share what they need—without repeating personal or sensitive information again and again
